[IRCServices] Possible bug but unable to reproduce

since i've been working with ircserivces for a while I myself have
seen such behaviour, i believe it was related to 3rd party vhost
modules for ircservices which sent CHGHOST to the server without
updating the fakehost string on the ircservices, and thus ircservices
are unable to detect the correct ban, or services believe ban is
already set while in reality the user is not really banned.
i can't be sure it was the issue, but it was the problem i had with
chanserv unban, and since i've patched the issue i'm pretty sure i
didn't see this no more.
if this is not the issue however it could be simply desync, happens a
lot, i get a "user parted but never joined" message in the logs almost
every week, shit happens on multiple server networks.
if unreal was coded as well as ircservices i'm sure none of this would
have happened :)
