Where is the third ban here ?

Where is the third ?

You only do set 2 each time, your ircd seems to be buggy, and it thinks
you were sending 3.

Since services gets also 2, and after 6 times, where only two bans are
set, it removes 12.

There is no problem I, unless you can say, where the third ban is.
