WE had these problems, this is how we solved it,

for the A kills/Glines, we re-started the services, and addded L and H lines
to the config?? Dunno.. but it fixed it.. so ????!!!
For the op/deop in channels #a and #b - the same settigns etc.. this was a
simple thing of re-stating the services again, and re-registering the
channels, Dropping them, re-starting services, and re-registering,

OperServ's CLEARMODES : - Again, the same solution...

ChanServ's MLOCK doesn't work at all. - and yet again... re-start..

I dont know what IRCD your using, But i bet it's UnrealIRCD,

re-write your config files for the IRCD and hte services, clean it out, take
out un needed lines and check for typos etc, We did all that, and it fixed
our problems...

It also has to do with if you hav multipule servers link4ed, you need U
lines for the server', Server <-> Server U: Lines,
