[IRCServices] Problem with v5.0.11

We're running ircservices v5.0.11 with the latest release of TR-IRCd v5,
and  databases converted from Epona using the convert-db program, and are
getting the following error occasionally, at which point ircservices
crashes:

FATAL: introduce_user() loop detected

Unfortunately, I haven't been around at the times this has happened to
 see exactly what was done, but I'm running the services in debug mode now
and hopefully will have more information soon, if required. needed.
